Clark street, 6TH floor, chicago, IL. Contractors are required to submit a copy of their payroll to the contractor officer within seven (7) days after wages are paid to their employees. Additionally, please complete the attached GSA form 1142, release of claims which is required for payment processing. In order to be paid promptly, all invoices must be forward to our payment center shown in block #24 of this form. Also, invoice must reference the accounting control transaction number in block #4 of this form. Failure to provide act number on invoice may delay payment.
